Online Registration Instructions

OUTDOOR HOUSE LEAGUE

  

READ FIRST, IMPORTANT STEP

  • In order to ensure that registration confirmation are automatically emailed to you please add our site to your trusted sites, the url to add is:
    http://www.heartssoccer.com
  • To complete this step, please open our website, go to the tool bar, click on tools,click on security,click on trusted site option, click on sites,and then click on "ADD"  or type in the url.
  • In the event that you don't add this to your trusted site it could be blocked by your browsers popup blocker and delivery of registration form via email cannot be guaranteed. This is also true if you have inadvertently installed multiple popup blockers.
  • Please be sure to write down your confirmation numbers and the date you registered, please let us know if you have not received the email, you must also provide a valid email address.
  

A. Player Profile

  • From the drop down menu select the number of players you wish to register.
  • All personal information must be inputted for each player.
  • Any questions in red must be answered.

 

B. Parent/Guardian, Contact Profile

  • All requested information must be inputted.

 

C. Parent/Guardian Requesting to Coach

  • On the registration form, if you click yes to coach, a second window ”Request to Coach Form” will pop up.
  • Please complete the form and Click Submit.
  • Print the Coach’s Letter – Click Here.
  • A separate form must be completed for each team you are requesting to coach.
  • For each of your children’s team which you are requesting to coach, no form of payment is required.

 

D. Registration Process with a valid email address

  • When you have inputted all the requested information, please click Submit Registration.
  • If you provided an email address, an automatic email confirmation of your registration will be forwarded to you.
  • You do not have to print the Registration Form.
  • Choose your Method of Payment and complete the process.

 

E. Registration Process without a valid email address

  • Upon clicking Submit Registration, a confirmation of your registration will be downloaded unto your computer screen.
  • This is your confirmation page.
  • Please click VIEW  to access the  player Registration Form.
  • Please click on the Tool Bar, then click size, then click smaller in order to enable the Player Registration Form to fit on one page.
  • Print two copies of the Player Registration  Form(s), one for yourself and one for mailing(only if you are paying by cheque).

 

F. Methods of Payment

  • By cheque – print the name of your child(ren) on the front of the cheque and mail it to the office. No printing of form is required.
  • By cash – after clicking VIEW, print the player Registration Form and visit the office to pay.
  • By credit card, from the Confirmation Page click Credit Card, provide the required credit card information and submit the completed information online. No mailing is required,

G. Payment by Cheque

  • Please make the cheque payable to North York Hearts Soccer Club.
  • Insert your child’s/children name(s) on the cheque.
  • Mail the Player Registration Form(s) and cheque within 5 days of registering to

  • When your payment has been processed, you will be emailed a confirmation notice included with a receipt for registration fees paid. Please keep this for your records. No other receipt will be issued.
  • In order to receive a processed receipt, you must provide us with a valid working email address.

H. Receipts

  • If you want a receipt for your records, you must give us a working email address.
  • We do not mail receipts.
  • There will be a $10 surcharge for the issuance of any duplicate receipt.
